I’ve just finished a case that stayed with me. Not because it was straightforward, but because it wasn’t.
The patient initially arrived visibly nervous and brought her daughter for support. That alone told me this appointment carried real emotional weight. She had researched us extensively and came in with clear hope and expectations.
Her goal was simple and urgent: a fixed upper full arch implant solution, and she wanted to move forward as soon as possible.

We discovered she already had two upper implants placed more than ten years ago. The remaining upper teeth were failing. They were no longer functional or aesthetic and were beginning to affect her confidence and quality of life.
On examination, her lower teeth were stable and not a concern for her. The upper arch, however, was where both the functional and emotional challenges sat. She had multiple crowns, one of which had previously fractured and been re-cemented. Around the existing implants, there was visible gum recession, and imaging confirmed bone loss.
For many clinicians, this would be the point to remove everything and start again. Instead, we discussed a more conservative and carefully planned approach.
I explained that we use Straumann implants, which come with a long-term guarantee, and that in suitable cases, existing implants can be reused. I was also very clear about the limitations, the gum recession, and the need for ongoing monitoring. Transparency is essential, particularly with anxious patients.
Because of her anxiety, sedation was essential. For the procedure, we were joined by Dr Hannah Wilson, our lead sedationist, to ensure the appointment was calm, controlled, and as stress-free as possible. That support made a significant difference to the patient’s experience.
The treatment involved a full upper arch replacement while incorporating the two existing implants. At the same time, we carried out reparative techniques and grafting to address the recession and compromised tissues. This was not an easy case. The bone loss was severe, and the planning had to be meticulous.

That experience allowed us to adapt the plan rather than abandon it, and to work intelligently with what the patient already had instead of defaulting to more invasive treatment.
The outcome was deeply rewarding. Not just aesthetically, but functionally and emotionally. Watching the patient relax, place her trust in the process, and then see the final result was a reminder of why I do this work.
Cases like this aren’t about quick wins. They’re about listening, careful planning, and using experience to guide decisions when the solution isn’t obvious.
