
One of the most rewarding parts of my work is meeting and helping patients who have spent years believing there’s no good solution for their teeth. Recently, we finished treating a lovely lady whose journey has stayed with me, not just because of the clinical outcome, but because of the confidence she found along the way.
She travelled all the way from Sheffield with her husband, to see us. That’s never something I take lightly. When someone is willing to travel that distance, especially when they’re already anxious about dentistry, it tells me how much they’re hoping for a different experience.
From the moment we met, it was clear she had been through a lot. She had missing teeth, several loose teeth, and had struggled with dental problems for most of her life. She told me she never smiled with her teeth and felt embarrassed by her smile. Her teeth were naturally quite small, she showed a lot of gum tissue when she smiled, and she wished that wasn’t the case. She’d also been wearing a temporary denture, but couldn’t bite properly with it, so it never really gave her the quality of life she wanted.

Like many patients researching treatment online, she’d read about All-on-4 and All-on-6 implants before coming to see me. She was particularly interested in a fixed solution rather than a removable denture and liked the concept of an FP1 restoration, where the final teeth emerge naturally from the gums. However, she wasn’t sure whether her gums were healthy enough to make that possible.
Adding to all of this was a significant dental phobia. She admitted she was extremely nervous about treatment and hoped I’d have the patience to help her through it. Those conversations are just as important as any clinical examination. Before we talk about implants, scans or treatment plans, we have to build trust.
During her consultation, we used SmileCloud to create a digital smile simulation together. I find this incredibly valuable because it changes the conversation. Instead of asking patients to imagine what could be achieved, they can actually see a realistic direction for their future smile and become part of designing it. For someone who hadn’t smiled confidently for years, seeing that simulation was a very emotional moment.
After carrying out a full assessment, it became clear that advanced periodontal disease had severely affected the upper teeth. We discussed all of the available full-arch treatment options in detail, including the advantages and limitations of each approach. Importantly, we also looked at what could still be saved rather than assuming everything needed to be removed.

We provided a full-arch implant restoration for the upper jaw, where the prognosis of the existing teeth was unfortunately too poor. In the lower jaw, however, we were able to take a more conservative approach. By treating the gum disease thoroughly, we saved the healthy posterior teeth and only replaced the failing lower front teeth with implants. Preserving natural teeth whenever possible is always my preference, and in her case it allowed us to combine implant dentistry with successful periodontal treatment to achieve the best long-term outcome.
Because of her dental anxiety, we discussed conscious sedation as an option from the outset. Interestingly, as her confidence grew throughout treatment, she coped so well that she ultimately managed without needing it. That’s something I’m always pleased to see. Sometimes knowing the option is there is enough to help patients feel in control.
Once healing was complete, we finished her treatment with professional tooth whitening to brighten her remaining natural teeth and create a harmonious result alongside the implant restorations.
Looking back, what stands out isn’t just the technical side of the treatment. It’s the transformation in her confidence. At the beginning, she told me she never smiled. By the end of treatment, she was smiling naturally without thinking about it.
Cases like this remind me that implant dentistry isn’t simply about replacing teeth. It’s about listening carefully, understanding what’s most important to each individual, preserving healthy teeth whenever we can, and helping patients regain confidence that they may have lost years ago.
Seeing someone travel home not only with a healthier mouth but with a genuine smile makes every step of that journey worthwhile.
